WELCOME TO THE UNIVERSITY STUDENT DENTAL HEALTH PLAN
The University Student Dental Plan is a dental discount program provided directly to you by our dentists. We are not an insurance company, and there is no insurance company involved. This means lower administrative costs and higher savings for our patients. Members of this plan simply receive discounted prices on dental procedures ranging from 20-80% off. The discounted prices for our services are provided to you in advance so that you know exactly how much you will be paying for the treatment you receive. You are guaranteed the same price for the same service every time.

ELIGIBILITY
An eligible Member in the University Student Dental Plan must reside in Hawaii, pay the applicable membership fees, and fall within one of the following categories:
- You are a student enrolled at least half-time in regular credit programs at a University; or
- You are a student enrolled for credit in a continuing education program,
postdoctoral fellow, or participating in a University sponsored
program; or
- You are a visiting colleague with a non-compensatory appointment.
Dependents Coverage. Dependents are also eligible to enroll in the Plan for the same benefits and period as the Member as long as they are the spouse, domestic partner or the unmarried dependent child under the age of 19.
Payment and completed enrollment form for the student must be received before newly eligible dependents are added on the University Student Dental Plan.
Choice of HFDC Dentists and Locations
There are ten (10) HFDC locations on Oahu, Maui,
Kauai and the Big Island at which a Member may receive dental treatments.
Members covered under this plan must see an HFDC provider. HFDC
has thirty-five (35) dentists and specialists to choose from. For
a complete listing click here. It is highly recommended that Members schedule appointments ahead of time
for treatment, especially if a specific dentist is being requested
to provide the services.
Dental Services Provided Under Program
Each Member shall be entitled to the dental services
set forth in the Copayment Schedule. For a complete fee schedule
click here. All copayments for services are to be paid directly to the dental office
by the patient, and will be collected at the time service is rendered.
There is no maximum benefit dollar amount for dental services received
under the program.
Membership Card
Upon joining the Plan HFDC will mail each Member a Membership Card. The Membership Card is proof of membership in the Plan and must be presented at any HFDC location at the time of the visit. Lost cards should be immediately reported to HFDC so that a replacement Membership Card may be issued. A minimal fee to cover issuance of a replacement Membership Card may be charged by HFDC.
Limitations and Exclusions
The following dental services are specifically excluded from the Plan:
- Select cosmetic procedures
- Prescription drugs and/or the dispensing of drugs
- Treatment rendered outside of an HFDC facility, i.e. services performed in a hospital
- Temporomandibular Joint Dysfunction (TMJ)
- Treatment of jaw fractures or dislocations
- Treatment of cysts, tumors, neoplasms or congenital malformations
- General anesthesia including intravenous or inhalation sedation
- Specialist consultation for non-covered dental services
- Any dental service not specifically listed in the Copayment Schedule
Effective and Termination Date
Coverage becomes effective on the first day of the
period or the date the application and full premium is postmarked,
whichever is later. Coverage terminates at the end of the period
through which premium is paid. Dependent coverage will not be effective
prior to that of the Member. Coverage is available only in Hawaii.
There shall be no refunds given for any cancellation from the plan.
You must meet the eligibility requirements listed in the Eligibility
section to continue dental plan coverage. To avoid a lapse in coverage,
your premium payment must be received within 7 days after the date
your coverage terminates. It is the student's responsibility to
make timely renewal payments to avoid a lapse to coverage. HFDC
will send renewal notices, prior to the Member's termination date,
to the address on file.
Termination of Membership
HFDC reserves the right to terminate a Membership for any of the following reasons:
- Member no longer meets
the criteria for membership.
- Member fails to make a
required payment when due.
- Member fails to pay the
copyament fees for dental services received.
- Member engages in condunct
that hampers HFDC's ability to provide dental services.
- Member commits a fraudulent
act to obtain benefits under the Plan.
Complaint
Resolution
In the event that a Member has a complaint or dispute as to a charge or
service provided under the Plan, the Member must submit a detailed
written explanation of their complaint to HFDC administrative office.
HFDC administrators will review the complaint and provide a response
to the Member as to their recommendation for resolving the complaint.
Any complaint or dispute by a Member which cannot be resolved must
be submitted by the Member only to binding arbitration in Honolulu,
Hawaii, within one year of the date of the act or occurrence from
which the complaint or dispute arose.
