UNIVERSITY STUDENT DENTAL PLAN COPAYMENT SCHEDULE FOR
2008 - 2009 All HFDC services must be provided or arranged by
a network provider. Members of the University Student Dental Plan
are not subject to waiting periods or annual maximums.
For NEW PATIENTS: A comprehensive exam and full mouth series of
