TO THE HO`ALA PROGRAM
Hawaii Family Dental Centers (HFDC) is proud to introduce Ho'ala,
an alternative program for the dental care needs of Hawai'i's people.
Ho'ala is not an insurance plan, but rather a membership program
that enables you to receive complete dental care at discounted rates.
Ho'ala was created especially for individuals without dental insurance.

HFDC was established in 1986, and since then has grown
to ten (10) convenient Centers statewide. HFDC has every specialty under
one roof. This means that you only have to visit one HFDC location
to meet all your dental care needs.
What
Can You Expect by Becoming a Member?
Quality is never compromised. Ho`ala is a program
that values traditional patient-doctor relationships. You are free
to choose your own dentist from a group of highly qualified licensed
professionals.

Stress or Worries About Going to the Dentist
Once you have chosen your Center and doctor, then the rest
is up to us to provide the quality, clean, comfortable, and gentle
care that you can expect.

Low
Membership Fees
Because Ho`ala is not an insurance plan, there are no claim
forms to complete or billing to be done. This means cost savings
that we can pass on to you through low membership fees and low out-of-pocket
costs.
Fixed
Out-of-Pocket Costs
We offer fixed discounted rates on all dental procedures.
All discounted rates are provided to you in advance so that you
know how much you will be paying for the procedure you receive.
This means fixed out-of-pocket costs for you and your family.

Annual Dollar Maximum
Insurance plans have a $600, $1000, or $1200 annual maximum.
Any major dental work such as dentures or crowns can easily exhaust
the annual benefit limit. As a member of Ho`ala, you are not restricted
to a maximum dollar amount.
No
Restrictions
Most insurance plans have restrictions and conditions that
apply mainly to large procedures. For example, if you break a crown
by accident, insurance will not cover the replacement crown if the
crown is less than five years old. In this situation, you would
have to pay for the crown at full price. Under the Ho`ala Program,
you are offered the same discounted price for your replacement crown
every time.

Exclusions
Crowns, bridges, orthodontics, and cosmetic procedures are
a few of the services available to Ho`ala members.

Waiting Periods
With the Ho`ala Program, there are no waiting periods.
You are immediately eligible for any dental procedure.
Discounts
on Orthodontics
Our comprehensive orthodontic work is offered at a 20% discount.
An average savings of $1000-1500.
Discount
on Teeth Whitening
We offer two teeth whitening programs, Nite White and BriteSmiles.
Ho`ala members will receive a 10% discount on these services.
HOW
DO I ENROLL IN THE HO`ALA PROGRAM?
Stop by one of our HFDC locations and pick-up an enrollment
form or call (808) 523-3103 to request one or
click here to
enroll online.
Fill-out a Ho`ala Program enrollment form for you and any other family member.
Send in your enrollment form and your annual membership fee, with a check
made out to Hawaii Family Dental Centers, in one of the following
amounts:
- Singles $192
- 2-Party $384
- Family (3+) $672
You will be sent a membership card in the mail approximately one week following
the receipt of your payment.
You are eligible to begin using services the first day of the following
month.
Call the HFDC location of your choice to schedule an appointment.
Your fixed out-of-pocket cost will be collected the day you receive services.
