Back to Top Can I choose a dentist?
As a patient of Hawaii Family Dental Centers you will have the freedom to choose any dentist at any location, anytime. If you are a new patient you can see the first available dentist, or you can select a dentist from our HFDC provider network. It is important that you select a provider that meets your needs as a patient.

Back to Top Can I go to different HFDC locations?
Yes, patients of HFDC have the freedom to go to any one of our 10 locations. Patient coverage also applies on the outer islands, which means if you are traveling and have dental needs, you can visit any HFDC location.

Back to Top What happens if I miss a dental appointment?
We request that our patients call at least 24 hours in advance to cancel an appointment.  Appointments that are cancelled with less than 24 hours notice are considered Broken Appointment and may be subject to a cancellation fee.

A Broken Appointment is defined as the following and is subject to a cancellation fee:

  1. Not showing up for an appointment
  2. Canceling a schedule appointment with less than 24 hours notice

Back to Top What is the Broken Appointment policy?
Following is HFDC's Broken Appointment policy. 

  1. 1st Broken Appointment - The patient is charged a cancellation fee.
  2. 2nd Broken Appointment - The patient is charged a cancellation fee and a deposit for future appointments is collected.
  3. 3rd Broken Appointment - After three Broken Appointments within a 12 month period, the patient is subject to dismissal.

We ask that you cancel your appointments with us at least 24 hours in advance. We will be more than happy to re-schedule appointment for you at a more convenient time.
  Appointments that are cancelled with less than 24 hours notice are considered Broken Appointments and may be subject to a cancellation fee.

HFDC accepts the following forms of payment:

  • Cash/Check
  • Credit Cards: American Express, Mastercard, Visa, Discover
  • Unicorn Financial Financing Program

Back to Top Do you have financing?
Yes, you may finance your major dental work through ChaseHealthAdvance. ChaseHealthAdvance offers a 12-month, 18-month and 24-month plan for eligible services such as Fillings, Braces, Bridges, Implants and Re-constructive work.   Applications for financing require a minimum treatment amount of $300.00.  You can apply for financing at any of our HFDC locations, or on-line by clicking on the banner below.

Yes, we accept new patients on an on-going basis. Please call the HFDC nearest you to make an Initial Exam appointment.

If you are a new patient you will need to bring in:

  1. Driver's License or State ID
  2. Insurance Card
  3. Patient Information (optional)

Back to Top What if I do not have dental insurance?
Patients without dental insurance will be responsible for payment (in full) on the day services are rendered.  HFDC also offers dental coverage for individuals not covered by an insurance plan.  If you are interested in the HFDC's Ho'ala Dental Program, please call us at 523-3103.

Back to Top What happens if I have an emergency?
HFDC provides 24-hour emergency care. If a dental emergency arises during normal business hours please call the location nearest you to schedule an appointment.

If you require emergency care after normal business hours, please call the Emergency Care pager number listed under your HFDC location.

Charges for an after-hours emergency visit is $160 for the exam and x-ray.  Any other services will be an additional charge.  All fees are due at the time of the emergency visit.
